Supporting Innovation in Primary Care

The Linde Family Innovation Grants Program in Primary Care is part of the Linde Family Institute for Primary Care at Beth Israel Deaconess Medical Center (BIDMC).

Through a generous gift from the Linde Family Foundation, this program within BIDMC Medical Education will provide an opportunity for physicians and staff in BILH primary care practices to develop a solution that will strengthen primary care practices in support of the Triple Aim — improving the patient experience of care, improving the health of populations and supporting high value care — and/or provider and staff wellness.

The Linde Family Innovation Grants Program in Primary Care will fund three to four projects in the range of $5,000 - $30,000 per project over a one-year project period.

Potential Project Examples

1. Expansion of e-consults in the BILHPC Needham region

  • Setting: BILH Primary care
  • Budget: $14,998

2. Improving clinic documentation of home blood pressure readings through use of two-way text messaging in primary care

  • Setting: Healthcare Associates
  • Budget: $23,800

3. Expanding diabetes care through pharmacy lead intervention

  • Setting: Charles River Community Health
  • Budget: $29,637

4. Reducing overdose deaths via primary care naloxone education and distribution

  • Setting: BILH Primary Care (multiple locations)
  • Budget: $8,239

The Grant Process

Eligibility
  • Open to all MDs, NPs, PAs, nurses, social workers and administrators working in primary care practices across Beth Israel Lahey Health primary care.
  • Applications from multidisciplinary teams, as well as those that include patient partners are encouraged.
  • Experience with quality improvement is an important consideration.
  • Applicants and team members should have the skills, capacity and experience to deliver the proposed innovation and evaluate its impact.
  • Multiple applications may be submitted from the same practice.
