Access Health Information at BIDMC
Health Information Management at Beth Israel Deaconess Medical Center (BIDMC) is dedicated to maintaining your medical records and keeping your health information private and secure in accordance with federal privacy and state regulations.
We partner with Sharecare, a digital health and wellness company, to make accessing your medical records as easy and convenient as possible.
Obtaining Your Medical Records From BIDMC
You have the right to request copies of your records. Medical records will remain available for 20 years after the patient's discharge or final treatment. To obtain a copy of a medical record, an authorization to release protected health information must be properly filled out.
Patients or patients' legal guardians may submit the form in the following ways:
- Complete the Patient Online Access Form.
- Fax the authorization to release protected health information request to 866-710-0132.
- Mail the authorization to release protected health information request to:

Records will be delivered to the email, fax or mailing address specified on the authorization.

Health care providers, attorneys or non-patients may submit the form in the following ways:
- Upload a letter of request and a completed authorization to release protected health information to the Requestor Submission Portal*.
- Fax a letter of request and a completed authorization to release protected health information to 866-710-0132.

Imaging Records
To request copies of X-ray, MRI, nuclear medicine or other imaging results, please visit Imaging Test Medical Records.
Birth, Death and Marriage Records
For certified copies of your birth, death or marriage records, please contact the City of Boston Registry Division.
Record Fees
We reserve the right to charge a reasonable cost-based fee for producing and mailing the copies, pursuant to HIPAA 45 CFR,164.524. This rate depends on the size and delivery method of the records being requested. At no time will the cost-based fee exceed Massachusetts law (MGL Chapter 111; Section 70).
