At Beth Israel Deaconess Medical Center (BIDMC), we put patients at the center of everything we do. As part of our commitment to offering the best possible care, we created the Case Management Program.
Led by physicians and registered nurses, the Case Management Program ensures consistent, appropriate and safe care and discharge. Nurse care managers (NCM) offer services and support for patients who may have complex care needs and require extensive resources to manage their health.
Our nurse care managers:
- Assess patient and family needs.
- Consult with Primary Care Physicians and specialists.
- Collaborate with insurance-based case managers to develop a post-hospital treatment plan.
Our program is administered within the larger patient care services division and helps promote stronger working relationships among nursing and other patient care services.
The Case Management Program is staffed seven days a week and includes dedicated coverage in the emergency department.
