Communication About Your Patient's Care
Once a patient is under our care, the BIDMC hospitalist team keeps referring providers updated throughout the patient’s clinical visit. This includes timely updates on the patient’s progress, test results, diagnoses and treatment plans. We work closely with specialists across various departments to provide comprehensive care.
After the patient's treatment, we ensure a smooth transition back to their primary care provider or specialist. We will provide detailed discharge instructions, follow-up care plans and any other necessary information to ensure continuity of care. This collaborative approach improves patient outcomes and satisfaction.
