Preparing for Your COPD Clinic Visit

What to Bring to Your Appointment

Please check with your insurance company when scheduling an appointment; you may need an insurance referral and/or a co-payment.

Prior to your visit please have your pulmonologist’s office fax us the following:

• Most recent clinic visit note, including current medication list
• Most recent pulmonary function test (PFTs), both pre- and post-bronchodilator and 6-minute walk
• Chest CT (send the CD and written report) (if available)
• ECHO (if available)

Please remember to bring with you:

• Insurance card
• List of medications
