An online appointment request form will be available soon. In the meantime, new patients can schedule an appointment by phone.
Appointment requests are for non-urgent concerns. If you have an urgent medical concern, call 911.
New Patient Appointment Request

Breast Imaging Appointment Request
For New Patients:
- To schedule a screening mammogram, please call us at 617-754-9500, Option 2; or fill out the Request an Appointment form (please select “Other” from the “Department” menu and type in “Breast Imaging”).
- To schedule a diagnostic imaging study, please call your physician’s office first and ask them to request one with us on your behalf, and then make an appointment following the steps above.
