If you do require a referral, please make sure to contact your primary care physician as soon as possible as these can sometimes take a few days.
Co-PaymentsIf your insurance plan requires a co-payment at the date of service, it is expected that you will be ready to make your co-payment during the check-in process.
We accept credit cards, checks, and cash for your payments.
Please bring the following to your appointment:
- Your insurance card
- Your co-payment, if applicable
