Information for Patients and Referring Physicians

Patient Information

ATTENTION: We've relocated to new office space on the East Campus. Details below.*

Hours: 8:00 am - 5:00 pm, Monday - Friday (except holidays)

Insurance Referrals:

Please remember that you may be required by your insurance company to obtain a referral from your primary care physician ahead of time.

If you do require a referral, please make sure to contact your primary care physician as soon as possible as these can sometimes take a few days.

Co-Payments:

If your insurance plan requires a co-payment at the date of service, it is expected that you will be ready to make your co-payment during the check-in process.

We accept credit cards, checks, and cash for your payments.

Please bring the following to your appointment:

Your insurance card
Your co-payment, if applicable
A list of medications you are currently taking
All medical information regarding your current diagnosis, such as MRI, CT scans, doctors' notes, pathology reports, and/or slides. We do not need this information if your tests were done at Beth Israel Deaconess Medical Center or if your doctor is here.

Bilingual representatives are on staff. Please let us know if you would like one at your appointment.

*Otolaryngology and Audiology Outpatient Clinic Move

On Friday, June 26, Otolaryngology and Audiology outpatient clinics moved to a new clinic space on the East Campus. We relocated from the Lowry Medical Office Building (LMOB), 6th floor, to East Campus, Shapiro building, ground level. Download Campus Map here.

Parking: Beginning on Monday, June 29, patients may park in the Shapiro Building Garage. Learn more about parking and valet services at BIDMC.

Parking is available in the Lowry Medical Office Building garage, which is accessible from Francis Street on the West Campus of Beth Israel Deaconess Medical Center. View Campus Maps here.

We offer one-stop-shopping for you and your patients. Our services are available in one location, making it more convenient for patients--and easier for you to make referrals and communicate with your patient's care team.

Simply call our appointment number at 617-632-7500 to speak to a member of our staff, who will arrange an appointment for your patient with the appropriate specialist(s) or put you directly in touch with one of the members of the team.
