Important information for your appointment
Insurance Referrals
Please remember that you may be required by your insurance company to obtain a referral from your primary care physician ahead of time.
If you do require a referral, please make sure to contact your primary care physician as soon as possible as these can sometimes take a few days.
Co-Payments
If your insurance plan requires a co-payment at the date of service, it is expected that you will be ready to make your co-payment during the check-in process.
We accept credit cards, checks, and cash for your payments.
Please bring the following to your appointment
- Your insurance card
- Your co-payment, if applicable
- A list of medications you are currently taking
- All medical information regarding your current diagnosis, such as MRI/CT scans, doctors' notes, pathology reports, and/or slides. We do not need this information if your tests were done at Beth Israel Deaconess Medical Center or if your doctor is here.
Bilingual representatives are on staff. Please let us know if you would like one at your appointment.
