Rehabilitation Services at Beth Israel Deaconess HealthCare-Lexington provides outpatient physical and occupational therapy.
Rehabilitation Services at BIDHC-Lexington cares for patients from BIDMC and the local MetroWest area ranging from adolescents to seniors. We treat a variety of impairments stemming from sports-related injuries, orthopedic surgery, balance/gait impairments, pain syndromes, repetitive stress injuries, postural problems, and neck and back injuries.
Our committed staff provides excellent clinical care in a personalized, respectful manner. Our staff is comprised entirely of professional licensed therapists to ensure the highest quality of care.
Appointments & Referrals
To schedule an initial Physical or Occupational Therapy appointment, please call 781-528-2510 or email us.
Check-In:
It is important that you check in at our PT/OT check-in desk so we know you have arrived for your appointment.
- Please allow adequate time for parking and check in.
- Please be prepared to pay your copay if applicable. We only accept credit cards.
- Please arrive 15 minutes early for your first appointment.
- For new patients only, please fill out our health status questionnaire in advance of your appointment and bring it with you.

- We are closed on weekends and all major holidays
- Closed Martin Luther King Day, Columbus Day, Patriots' Day, Presidents' Day
Insurance Referral:
- Please verify the requirements of your insurance plan to determine if a referral/authorization is required for your plan.
- If a referral/authorization is required by your insurance plan and we have not received it by the time of your appointment, you will need to sign a waiver that guarantees payment.

Preparing for Your Visit
During your first visit, a thorough initial evaluation is performed with each patient in order to develop a specific, individualized treatment program. Our therapists will work with you to identify your goals.
What To Wear (Or Bring To Change Into):
- Patients with back or lower extremity problems should bring shorts or loose fitting sweat pants.
- Patients with neck or shoulder problems should bring a tank top or sports bra or can change into a hospital gown
Need To Bring:
- Physician prescription/order for therapy — if the order is not entered into the BIDMC computer system by your physician, a written order will be required from the referring provider.
- If one was not given to you at the time of your MD appointment, the doctor's office can fax it directly to the department at 781-528-2570.
- A current medication list to provide for your therapist.
Co-Pay:
- Copayment for PT and OT is collected when you check-in for your appointment.
Meet Our Rehabilitation Services Team
Katherine BraggKatherine received her Doctorate of Physical Therapy from Simmons University in Boston Massachusetts in 2017. Before that, she attended University of New England in Biddeford, Maine to receive her Bachelors of Applied Exercise Science. Her clinical interests include those with neurologic and musculoskeletal diagnosis, and in 2019 she received her orthopedic clinical specialty certification. She has worked both as an outpatient and inpatient physical therapist for BIDMC for the past year, giving her a background in most patient populations. Outside of work, she enjoys being active outdoors including participating in soccer, hiking/camping, and snowboarding, she also appreciates baking and cooking, while always looking for a new recipe.
Heidi Brincklow, PTHeidi graduated from Northeastern University with a Bachelor’s of Science Degree in Physical Therapy in 1993. Out of college she began working in outpatient orthopedics. Most of her career has been in outpatient Sports Medicine Rehabilitation. Heidi has worked with youth sports teams in education, stretching, and injury prevention. She has participated in prescreening sports physicals for Middle School thru High School students. She has special interests in shoulders, knees, and ankles. She has completed continuing education course work on manual therapy for shoulders, hip, ankle, lumbar, and sacroiliac joint dysfunctions.
Heather Broglio, PT OCSHeather has been practicing physical therapy since her graduation from the University Mass Lowell in 1989. In 1999 after completing a year-long residency at the Institute of Orthopedic Manual Therapy her career has been focused on incorporating manual therapy techniques into her treatment. These skills and special interest in sports and personal training has allowed her to excel at guiding her patients back to their sport. She is a Clinical Specialist in Orthopedics in our clinic after becoming Board Certified in 2007. She has been active in athletics her whole life and still plays organized soccer. Since the COVID-19 pandemic, she has been part of the long COVID multidisciplinary team at BIDMC specializing in long COVID rehabilitation.
Jason Chandler, PTJason graduated with his Doctorate of Physical Therapy from Boston University in 2022 and completed a 1 year fellowship program from the Institute of Orthopedic Manual Therapy in 2023. He's always been interested in the biology behind the human body, and in particular how it applies to pain, movement, and their relationship to everyday function. Jason has experienced physical therapy rehabilitation as a patient himself over the course of several injuries sustained during playing organized sports and participating in his various outdoor hobbies, and likes to apply this perspective to his own care in order to get patients up, moving around, and back to what they love to do as quickly as possible.
Judith Criscitiello, OTL/R, CHTJudy has been practicing as an OT since her graduation from the University of New Hampshire in 1989. She became a Certified Hand Therapist in 1995, which requires participation in extensive ongoing clinical training. Her professional interests include treatment of trauma, post-surgical, and overuse injuries. She enjoys fostering understanding through patient education, and incorporates a holistic approach to the management of injuries and pain. As a violinist, she also has an interest in working with musicians and has presented to the Boston Symphony Orchestra and New England Conservatory on injury prevention and management. Judy’s personal interests include yoga, outdoor/nature activities, playing violin/fiddle, cooking, and spending time with her family.
Johanna Guarino, PT OCSJohanna is a Lead for Physical Therapy at BIDHC-Lexington. She has been a practicing physical therapist since graduating with her Bachelor of Science degree from the University of Massachusetts, Lowell in 1989. In 1999, Johanna completed a year-long residency and received a Certificate from the Institute of Orthopedic Manual Therapy. In 2007, she received a certification as an Orthopedic Clinical Specialist which she recently recertified. Johanna also has been a teaching assistant at Boston University.
Paul Livolsi, PT, DPThPaul received his Master’s Degree in Physical Therapy in 1997 from University of Massachusetts - Lowell and then received his Doctorate degree in 2005. He has been practicing since 1998. His clinical interests include: Orthopedics, Manual Therapy, and Musculoskeletal Exercise Program Development. His experience extends from VA Healthcare System to private practice and now hospital-based community health center. Paul has been an active participant in clinical education for BIDMC system for students from the surrounding clinical institutions.
Noreen Straughn, OTL/R, CHTNoreen is a Lead for Occupational Therapy at BIDHC-Lexington. She graduated from the Occupational Therapy program at Tufts University in 1988. Noreen has been an Occupational Therapist on BIDMC’s Hand Service since 1989. She worked in Boston for 10 years beginning with inpatient orthopedics and then becoming the Outpatient Clinical Coordinator. She has been the lead therapist at BIDHC-Lexington since 2000 and is a Certified Hand Therapist. Noreen specializes in post-operative hand rehabilitation and treatment of repetitive strain injuries, including trauma. Noreen’s personal interests include biking, gardening, and cooking.
