Preparing to Go Home

When it is time for you to go home, we want to make sure you feel prepared. That's one reason our staff will begin talking with you about discharge early in your stay. Here are some general tips that can help discharge go smoothly.

  • Talk with your nurses and doctors about the medications you will be taking at home, and whether you need to limit your activity in any way. If you have questions, please ask.
  • Ask your doctor when you might be discharged. Contact family and friends well in advance to make sure someone can bring you home.
  • If you do not have someone availableto pick you up, ask to speak to a case manager. The case manager will talk with you about suitable means of transportation, such as a wheelchair van.
  • In most cases, discharge occurs by 11 a.m. Please relay this information to the person who will be picking you up.

Please tell us right away if you have any concerns about going home. By planning ahead, many problems can be solved.
