Social Service and Case Management
Receiving one-on-one attention is our specialty here at Beth Israel Deaconess Hospital-Needham. In addition to great medical care, our staff of professional social workers interact with patients and their families during their hospital stay. Social workers address and provide psychosocial support, crisis intervention, and referrals. Social work services are also available to patients seen in the Emergency Department and outpatient clinics. The Social Service and Case Management Department also maintains information about community resources and works collaboratively with various health care and human services agencies.
Discharge planning services are provided by the social workers and a nurse case manager. The nurse case manager consults with the clinical team and collaborates with the insurance case manager to facilitate the recommended discharge plan in accordance with the insurance requirements. The discharge plan will be developed with patients and their families based on the health care team's assessment of the patient's needs. The plan may include arrangements for home health care services, rehabilitation or nursing home placement, and the goal is to ensure a safe and clinically-appropriate discharge.
