• Monday, Tuesday, Thursday and Friday from 8:00 a.m. - 4:30 p.m.
• Wednesday from 8:30 a.m. - 4:30 p.m.

What to Bring to Your Appointment

Please bring with you to your appointment:

• A list of medications you take including prescription medicines, nonprescription medicines, vitamins, herbs and supplements, the dose and how often you take each.
• Any x-rays, CAT scans or MRI's you might have related to the problem for your pain specialist to review.
